The American Academy of Sleep Medicine (AASM) has a current "clinical practice parameter" which says a retitration isn't necessary unless the patient is still having symptoms of excessive daytime sleepiness (EDS) in spite of using CPAP routinely.
All other problems are usually related to the mask fit, lack of a heated humidifier, or someone simply not giving CPAP enough time and/or not being willing to adjust to it. These problems can and should be dealt with by a competent DME and sleep physician.
