The Nasal Aire is actually quite comfortable if it fits right. At first I tried a small and the cannulas when to far up into my nose and it hurt. The cannulas only need to fit just inside your nose. If it fits right it forms a good seal. You really can move around quite a bit. I am a side sleeper and I can roll around and it doesn't fall out. You just have one piece of velcro that you connect somewhere on the back of your head... where it is most comfortable for you. The exhaust ports are very small and point downward. You can't really "move" the ports in anyway to direct the exhaust. However, the exhaust is not all that "intense" of an air flow. Good luck to you.
