There's a very broad spectrum in what insurance companies will provide/pay for. In my case (HMO), I had no choice and the bottom-of-the-line CPAP machine was only loaned to me for the duration of my coverage. There was no option to purchase it and no provision that I would own it after a period of time, nor would they pay any portion of the cost if I wanted to buy one.
If there is a customer service number on your proof of insurance card, call and ask lots of questions. Depending on your options, it might be a good idea to take the bottom-line unit to start treating your sleep apnea while you're getting more information. You can always turn it in later.
I changed insurance when we had open enrollment and I'm now investigating my options with the upgraded coverage. I believe I'll be purchasing the autopap I want off the Internet and submit the bill to the insurance company for reimbursement.
