I agree about not checking your CPAP. The instruction manual that came with mine clearly warns that the included travel case will not protect it from rough baggage handling. Then as you point out there is always the risk of the baggage handlers losing it. I think I'd be very uncomfortable if I couldn't carry it on. I will remember to board early to ensure that my CPAP will be accomodated by some of that reserved space Reno Tom mentioned.
