I would only consider surgery after all other possiblilities have been resolved. You didn't say what type of mask or mask alternative you're using. You also didn't mention whether your CPAP or BiPAP had a humidifier and if so, was it a passover or heated humidifier. I would definitely experiment with all of these before I committed to going under the knife. Don't forget that surgery can have the unfortunate result of scar tissue which can lead you right back where you started.

Also, I have to question your doctor's use of a rather addictive sleep aid. If you use that, it should be used very temporarily (1 or 2 nights). Otherwise, you can experience worse insomnia coming off of it than you had before.
