Lets take things in order.
If you haven't had a sleep study since 1996 you are way way overdue. People with OSA should be seeing a sleep specialist every 6 months to 1 year, if things are stable.

CPAP machines can last 6 years, but the hoses, masks, etc should be replaced every 6 months. Please Please Please don't tell me you're using the same set up from 1996.

Even a little weight gain (or loss) can affect the pressure you need, hence another sleep study.

The odds are, even if your CPAP was working perfectly you would still feel tired and wore out. Adults need 8 to 8.5 hours of sleep every 24 hours. Adults with a serious medical condition may need more. You absolutely positively need more than the 6 hours you're geting now.

Not all pulmonologist know sleep medicine. Likewise, most sleep specialist don't know much about mets. You may need a tag team effort to get the best treatment.

Good luck, I wish you the best.
