Apneas are total obstrutions. Hypopneas are partial. The clinical definition is that a 90% or greater obstruction is an Apnea, and from 50% to 89% is a hypopnea.

Measurements indicating severity include the completeness of the obstruction, the number of events, the duration of events, and shortfall of oxygen in the blood. How high the CPAP must be set is not considered a measure of severity at this time. Also, how lousy you feel is not considered a measure of severity, as it's impossible to quantify.

BTW, 12 hyopopeas per hous means 96 events in an eight hour night. This is enough for CPAP.
