You mention when you attempt to wean yourself off the Pred you find yourself becoming as "depersonalized as before" and cant venture out due to fear of an impending blackout.
That sounds like an anxiety attack.
And to my unprofessional self, also implies addiction, because it happens when you try to "wean" yourself off.
So, I wonder if you may benefit from a good hypnotherapist, or therapy of some kind, to help with the anxiety symptoms and get to its underlying causes and to help get you off the Prednisone, which you indicate you expect is harmful. Anxiety can also cause fragmented sleep.
