When I contacted the online dealer via 800#, she said that the sizes are the same between the NA1 and NA2, but that the Medium Plus is somewhere in between the large and medium. If my CRT indeed had the correct size (medium plus), then the M+ is a lot larger than the M. At this point, I'm almost convinced that he had the large or extra large size for the NA2 which obviously did not fit me. When I was first fitted for the NA1, the large size fit in one nostril, but not the other. The M+ for the NA2 (according to my CRT) did not fit either nostril.

As far as lengthening of the tubing, from what I saw, it does not appear that they did.

Don't have an answer on the "wearing it above the head" question. Sorry.
