Posted by Recruit on January 09, 2004 at 09:21:27:Does too high a CPAP pressure cause arousals due to PLM's to increase? If so, would reducing the CPAP pressure some help PLM arousals? What's interesting is that a CPAP pressure of 6 caused my apneas/hypopneas (2/85 respectively)to be reduced to 0/3 even though the clinc continued to increase the pressure to 8. This is currently what my machine is set at. My overall RDI went from 15.0 to 0.5. The doctor indicated he wanted to increase the pressure further because I'm a larger person (6'2" and 220 lbs). Could decreasing the pressure from 8 to 6 in my case reduce my PLM arousals?
My initial (w/o CPAP) sleep study reveiled the following arousals/hr. (index)
Respiratory---13.8
Limb--------------0.5
PLM--------------0.0
RRLM------------0.0
Spontaneous---7.2Total-------------21.6
My followup (w/ CPAP) sleep study reveiled the following arousals/hr. (index)
Respiratory----0.5
Limb--------------0.9
PLM--------------6.1
RRLM------------0.0
Spontaneous--12.8Total-------------20.3
The CPAP all but eliminated the arousals due to apneas, but arousals from PLM's significantly increased. The doctor has prescribed Klonipin. Can't tell a difference yet since I've only taken the Klonipin for 2 nights now. Looks like I may be snoring more now too since spontaneous arousals are up.
What stinks is as a result, my total arousal index has only dropped from 21.6 to 20.3. No wonder I'm still tired all the time!
