The way I read Guilleminault is "Time spent above 1500 meters can excerbate OSA."
The way I haave analyzed the situation is as follows:
The airway is kept open by the difference in presssure between the environment (Tip of nose) and the interior of the lung. When this is insufficient, CPAP raises the pressure at the tip of the nose to a level adequate to maintain an open airway. Going to the higher altitude lowers the pressure at the tip of the nose.
This is similar to my line of reasoning that exercise lowers the pressure within the lungs.
Of course I am not a doctor, evaluate the above for yourself.
