I wouldnt worry too much about alleviating your symptoms before going to a sleep clinic for fear of wasting their time. From your description of your wife's comments, I would bet that it is very likely that you have apnea, and that's what they're there for, to treat apnea.
Untreated apnea has a terrible effect on the heart, as does smoking, so smoking obviously doesnt help your breathing. And at its root, apnea is essentially a breathing disorder
Yikes! five to 20 cups of coffee a day! If I did that, I would be on the ceiling! Excessive caffeine does in fact have a highly detrimental effect on sleep -- especially if you drink it late in the day or in the evening
Do you also have insomnia?
They will probably have you take a sleep study, and I suspect they will find you do in fact have it and prescribe CPAP.
