I quite agree the the frame of reference for people who have had SDB for 10-20 years can be somewhat skewed and that's why many have low subjective sleepiness (there are some studies to show exactly this phenomenon- but they are still in clinic patients). The problem remains that there exist in the population enormous numbers of people at greater risk of cardiovascular disease that we do not currently have a solid idea what to do with. APPLES migh help with this but then again it might be a negative trial in the subgroups we're discussing (high AHI low sleepiness). It will also probably show little benefits in treating people with mild OSA (low AHI High Sleepiness) with CPAP as well.
When are the APPLES results due to be presented? Denver or the year after?
