To possibly answer your questions regarding the different pressures - are autopap machines to be trusted? To me, it is as close as you can get to the real thing with out having to have all of the wires on in a strange place, though not completely accurate.Did you get any meds to help you go to sleep while at the sleep center? The ideal is to have the full "hook up" while you get titrated; this insures proper pressure settings for your body, evaluated by a person not a machine.
Is the differential due to the mask? Shouldn't the pressure be the same for the two?
It is possible that the nasal mask I prefer is a respironics comfort gel. If you are opening your mouth while you are on the auto-c, it is possible that the machine is compensating for what it sences as airflow limitations. You may wake up when the pressure increases.
Do you still have your tonsils and adenoids?
B) regarding why I am not feeling better.
My best guess is that you are not feeling better because you haven't gotten an accurate pressure set for your needs and your sleep is fragmented. Do you still have the full face mask? Maybe you could try it again. I bet you are opening your mouth in your sleep.
Is there any way they will let you try another over night cpap psg? I think it would be beneficial.
As far as pressures go, sometimes it is necessary to switch over to BiPap once a certain pressure is reached generally around 15cm H2O.
If you want to/get to go back you will know what to expect and maybe you can get your doc to RX you a sleep aid so you can sleep and get titrated by a qualified person.
