I have been on bipap for a year and a half, and always use ramp. My pressure has been 18/13, but was changed this week to cpap 13. If I don't use ramp I can not get a good seal. It is like the air has to open up the air way. Maybe if I would turn it on while still sitting up it might do. I do wear nasel pillows, so that may be part of it. But, I think the few times I wear a nasel mask it is the same way. The pressure itself doesn't bother me, just the leaks.
