As far as the problem with nasal pillows, I'm assuming that we're talking piercing the side of the nose, not between the nostrils. The air would simply run out the hole!

Also, a hole on the side of the nose would need to be inside the seal of the mask to prevent the same problem. What’s inside with one style of mask might be outside with another.

I don’t think that a hole between the nostrils would in any way compromise the seal, but I’m not certain about that.

I guess that I forgot to add that you would have to remove whatever jewelry you put in the piercing; it would almost certainly be in the way of the mask!

In conclusion, you should not pierce your nose or wear nose jewelry, to avoid potential problems using CPAP. People seem to have enough problems using CPAP without adding new ones.
