Insurance companies view APAPs as Auto CPAP's and pay them out that way. If you are working with a DME, they don't like giving out APAP's as their profit margin is thus lower.

As long as your doctor states on his script a auto cpap your insurance may cover it. All you can do is have your doctor write the script and send it in for approval.

If you need to battle your insurance company tell them an auto cpap is gonna be cheaper than a new sleep study when your regular cpap doesn't cut it.
