..breath, Santa and Boston provide excellent advice. Your surgeon is quick to operate and slow to listen ie the comment regarding exhalation. It's just a shot in the dark but my experience may be similar to yours or at least helpful. I have had difficulty adjusting to PAP/masks. When I started PAP, I would awaken after a couple of hours with difficulty on inspiration(inhale). When I removed the mask I could breathe through my nose easily. That didn't make sense, but the ENT had noted I had a bit of a nasal valve. As it turned out, the mask and PAP pressure actually pinched my nasal passages shut, rather than open. Even with PAP, I used breathe rite strips. I know this isn't quite what you describe but maybe it will help. Good Luck and absolutely get a second opinion before surgery.
