Conventional wisdom is to be retitrated after a loss of 20 pounds so I would say that an autopap would be a wise way to go. I recently adjusted mine downward.

Taking sick leave has to be your call; however, just because you're able to drag yourself into work does not necessarily mean you're doing an acceptable job once you're there. In any event, it sounds like you need some rest.
