Your CPAP should always be a carryon item. Security has never questioned it-I just put it on the belt with my other items, like my purse.
However, once on a small commuter jet, I was asked to check it becase it did not their meet very small carry on size. I insisted that it had to be a carryon item since it was medical equipment. The stewardess then put it in a special in-plane bin at the front of the cabin.
You can always ask your physician to write out a note to bring when you travel to verify your need of CPAP.
