At Salt Lake City, on a regional connector, I was TOLD, as we were going up the outdoor jetway,
I would have to put my little wheel-on which contained my CPAP equipment into checked luggage, because of space.
I said "I have a thousand dollars worth of medical sleep equipment in here with my other personal carry-on items, and it is staying with me on the plane". At that point,
they found that little space at the front of the plane. I wasn't nasty, just stood my ground.

I pack the CPAP in with my little pillows, some soft items, hose and Nasal Aire II. That avoids the liklihood of forgetting I have that extra carryon to keep track of, along with my purse, and little camera bag. My carry-on is sturdier than that bag that came with the equipment, anyhow.
I had information from my doctor stating my machine is medically necessary, too.
