English is the internation language of aviation. Probably no need to have a letter in Spanish. I don't know what your question about insurance refers to? Do not, under any circumstances, check your CPAP equipment. You want to carry this on board the airplane with you. If it is lost, damaged, or stolen during your vacation, you would want to check with your home-owners policy to see if such things are covered. It would probably be seen as no different than a camera or laptop computer as far as insurance is concerned.
