I do not work in the medical or sleep field, but in all the time I've been reading these internet forums I've never heard of someone being prescribed a CPAP with a pressure as low as 4 cm. Your situation presents many challenges, seemingly. I would take it for granted that you cannot tolerate a pressure higher than 4 cm and that not having the CPAP (on at 4cm) causes arrousals or apneas.
A logical fix would be to have the machine set for 6 cm and have it blowing a little harder to vent the CO2. Perhaps a Respironics CPAP machine with the C-Flex feature would be in order? Such a machine could be set at 6 cm and feel like it is lower due to the C-Flex feature.
You need to see your doctor again, though. The medicine (CPAP) that s/he prescribed is not working.
