I have lived in three cities in the last 11 years that I've been on CPAP and I can tell you that each sleep doctor I was working with had a preferred local home equipment provider (DME) and the mark-up with similar to what you describe. There was not, however, a CPAP store within the doctor's office or sleep lab.

I will take the opposing view to your post, though, and contend that the price paid to a local provider is VALUE ADDED and probably not out of bounds considering staff is employed to be of assistance for any patient, at a phone call, and your purchase probably provides you with a same day replacement policy if anything went wrong, such as accidently dropping the CPAP machine on the bloor and breaking it or breaking a mask. Try to get same day replacement or same day warranty repair of a CPAP over the internet. It wont happen. This added value can be important to some patients and is nothing more than people providing a service. It probably shouldn't be maligned.

That all said, I purchase all my equipment over the internet and then submit the claim to my insurance. I don't believe in over-billing insurance. But since I have a back-up machine and mask at home, the same day serivce is not needed.
