The mail order providers can indeed ship next day if needed, but they would be shipping a second purchase and probably not a loaner. I can't imagine any mail order (internet) company would be willing to send out a loaner overnight to someone in a distant area, expect to get the broken one back, then when that broken one is repaired, ship it back to the patient and expect to get the loaner returned. What if the person didn't send the loaner back? Their margines are so slim, they'd lose their shirt.
With a local provider, the patient would receive a same day replacement, but not if the first machine wasn't presented at the time. Then if that loaner was not returned, the local provider wouldn't be out serious money since they made 2X on the original sale and have the first machine (now repaired) to resell or loan out to someone else.
