I suggest you get a qualified sleep doctor. If the sleep center can't give the name of one, then, I suggest going through the American Board of Sleep Medicine (www.absm.org/Diplomates/listing.htm) Most regular general practioner MD's just don't have the specific knowlege on sleep medicine. You need to have the sleep doc write a prescription that says "mask to fit for comfort and effectiveness". The vendor that gave you a mask obviously didn't give you one that was effective. Therefore the insurance company shouldn't pay for the first one, or the second or however many brands, types etc it takes to get you the right one.(There are dozens and dozens of the maks out there.) You have to get a doc on your side and then don't take "no" or "sorry" from any vendor or insurance company. You may have to get very persistent or they may not give you the medical treatment that you need. But, the first thing is to get the sleep qualified doctor. Also read the links and postings on this website. The more knowledgeable you become, the more likely you will get help. Good luck and DON'T give up hope.
