job hunting advice

Has anyone with diagnosed sleep apnea looked for a new job? I am looking for a better job but I am nervous about what to put on the forms that ask for a health assessment. Compared to the way I was, my health is greatly improved. However, I have several medical conditions that are being treated successfully.

Any advice?
