You have my utmost sympathy!! I am with you in not wanting to start with medication on a 3-year-old, but I really have no solid advice for you. Why not take a chance on the CPAP? It might be that her brain would soon recognize it was getting the necessary oxygen and she would accept it. Perhaps you could slip it on her soon after she's sound asleep? (My daughter slept like a rock, so it might have worked with her.) Sounds like a pretty lame suggestion, I know.
Several forums back a woman posted regarding sleep problems with her teen-age daughter, another horror story, and her lengthy steps to resolution. Unfortunately, my search feature does not seem to be working since I reverted to dial-up and I couldn't find it for you. It seems she somehow found or was referred to a pediatric sleep specialist (I think) and I thought the post might give you some idea as to where to look next.
