Follow up more closely with your doctors or the dentist you saw who made the dental appliance for you. They are service and care providers and we here on the forum are not. You may get advice on what to do (here on the forum), and that advice could be suggestions to have another sleep study, or to try a different type of CPAP, or to have surgery. But, if you have no provider who will actually write a prescription for these services, then you're out of luck. Difficult cases should always be followed up on with medical professionals.
