To be constantly tired to that degree is symptomatic of some other problem, commonly sleep apnea, but it could also be something else. You do need medical attention. And if your daytime fatigue is sometimes "quite overwhelming," it's likely that it is not always under your control. The body begins to grab sleep in microseconds and that's dangerous on the highway. I once blacked out on the freeway for about five miles; no, I don't drink, and it was only by the grace of God that I didn't have an accident. To this day, I don't know how I navigated those five miles.
