FYI, there are well qualified oral and maxillofacial surgeons in Halifax who are affiliated with Dalhousie University's faculty of Dentistry. Surgical team is Dr's. G. and Dr. M and Dr. P. They are doing the same operation as is being done at Stanford, with good results. I got by asking Oral surgeons from U of T and UBC who was tops in this area.
