81 events and hour is EXTREMELY severe. 5-15 is mild, 15-30 is moderate and 30+ is severe. 81 is certainly excessive.
I had apnea at 37 events an hour and my situation was bad. I have had multiple surgeries which have improved my siutation although I continue to battle for different reasons that may not be applicable to him.
Surgery may be necessary because someone with apnea that extreme may not respond entirely to CPAP.
If he had apnea when he was thinner and younger it's probably not related to his weight (although it will exacerbate the apnea). He either has a very recessed jaw and or chin or excessive tissue in his mouth/throat. They may possibly advance his jaws (which I had done; called maxillomandibular advancement {MMA}, or they may cut some soft tissue out).
