Your situation is VERY similar to mine. The main exception is that I got to this question in 5 years, not 12 as you.

My final question was exactly yours also: Trach or MMA. I was ready for WHATEVER the solution was going to be. I could NOT continue as I was.

I flew from TX to the Bay area to see Dr NP for MMA. The rest is history. See my blog and feel free to ask questions.
