I would absoultely consider MMA as a surgical option. It's not as bad as it sounds -- I have had it done. Read more about it in the non-cpap options forum. I wouldn't go with the trach for multiple reasons...use that after you have exhausted other surgical options.

In the meantime, if air is escaping through your mouth at night it will lower the efficacy of your CPAP, which would explain why you are still waking up with symptoms of apnea. Of course, you have the option of using a chin strap to prevent this from happening --- I actually duck taped my mouth shut! I used the same tape that you use when you paint walls. Works better than the chinstrap. Sometimes you have to get creative.

Also, you may want to a BiPAP Auto (if your insurance promits), as it will adjust to different pressures every night, as needed.
