Yes, in theory, APAP's should adjust up and down according to needs during the evening and it should not stay at max, but in reality, different manufacturers use different programming and one machine may read the same person differently. I'm curious why you switched from the P&B to the Respironics? Did you feel you were not getting good results with the P&B 420E?

On other message boards I have read that the Remstar M series has had some production problems. You may simply have gotten a bad machine. Or, it could be working properly and it just senses that you need pressure.

Depending on where you purchased you M series, you may be able to return it and try another one to see if the problems go away.

Here's the bottom line: How do you feel the next day? Do you feel that the Respironics has given you good rest even though it goes and stays high most of the night? Do you feel it gives bad results? You have to be practicable about results with these kind of machines. APAP's are only for people who do fairly well on straight CPAP but are looking for better results. Better results may mean being able to tweek the settings to see where your body best likes the therapy pressure. Sometimes with auto's, moving the bottom or lower limit higher and making the range more narrow can work better than a wider range. Lowering the upper limit can help with run away pressure, but never set the upper limit to lower than your standard titration pressure.

Don't expect miracles with any APAP. Just expect things to be a bit better than straight pressure. If you never achieve better results over and above straight pressure, go back to straight pressure.
