I can tell you about Japan. I lived there when I was diagnosed with OSA. In Japan, the standard is an AHI of 20. I received a Resmed APAP machine subsidized by Japanese National Health Insurance.

I recommend you read the book "The Promise Of Sleep", by William C. Dement. Dement, along with Dr. Christian Guilleminault are the inventors of the AHI scale. They are the ones who decided on the designation of five as the point where OSA is clinically diagnosed. Interestingly, you will see that Dement thinks that people should seek medical treatment when their AHI stat is greater than five. He would certainly conclude that 40 is much too high.
