I'm sorry to read that they couldn't get you titrated in one night let alone TWO!
Not to berate hospitals doing sleep studies, but my personal experience is that hospitals seem too unconcerned with the patient's well-being when it comes to sleep studies. That was my experience. I prefer private sleep clinics.
They only moved you 5 cm each night. That's odd. When I was titrated during a split study, the tech had 4 pressures that the doctor could have prescribed. One of them was a 15. They tried as high as 18. I wound up with a 12.
I would ask them why they only moved you 5 cm each night. Could be hospital policy, insurance pays for two nights then will pay for in-home titration, maybe the tech isn't too knowledgeable about titration? Lastly, your case could be a difficult case to titrate. XPAP therapy and titration are not necessarily exact sciences. ;-)
I would also call the doctor's office and request that they have the PSG's from both night's for your perusal and ask the doc to explain them. Don't settle for the summary report.
One thing to keep in mind is that the doc's know how to play the system both for your benefit and theirs. Some doctor's play the system for their benefit only, and others play it for both. Just remember your needs come ahead of his/her paycheck! Be sure to get what you need. Insurance (if you have it) HATES paying for apnea patients. I don't understand why, once we get settled with the equipment and are responding well to proper therapy, its cheaper than BP meds and Diabetes supplies.
14 is where they usually put you on a BiPAP at minimum. I'm a little surprised that they put you on APAP with A-Flex already.
72% is very low and dangerous. I've read of people hitting 54% though. Mine was 84%--still not good, and with an AHI of 82.
I can relate to the low minimum pressure setting. For me, I need a 10 or higher--otherwise I'm gasping for air!
