Marco,

I went to a well known medical school’s sleep lab in Baltimore with strong faculty and do not think there’s any misfeasance re insurance etc. I think the problem with the PSG and 2 titration tests is a combination of just being uncomfortable with so many wires attached. I also had significant mask leaks both nights and poor sleep profile. Just not enough hours of sleep at a high enough pressure to validate the tests.

The sleep lab’s CRNP ordered an Auto M with C-Flex, but I think the DME chose A-Flex to facilitate the at home auto-titration testing. They were going to move me to a C-Flex after the first round of in home auto-titration, but I asked to keep the A-Flex and they agreed – possibly because it makes buffers the inhalation and that might make it easier to tolerate 18+ cm. We’ll see what’s going on in about two weeks.

Three questions:

1. I have been waking up between 2-4 am with moderate discomfort in my chest and lower lung region. This is when the APAP is usually pegged at 17 cm. Do you think this is caused by breathing deeply for the first time in ten years? Does it go away?

2. The other problem is I had my uvula removed surgically maybe ten years ago. I feel like my abdominal cavity is filling up with air and get a “bloated” feeling. Do you think the higher pressures are forcing some air down the wrong opening?

3. Do you think a BiPAP would provide better therapy than my APAP? It appears there are some advanced BiPAPS that deal with central/complex/mixed sleep apneas.

Steve
