APAP's are for people who already do ok on CPAP but are looking for better results. I think this is a valid assumption for UARS as well. I don't believe there are people with UARS cases that do not respond to CPAP but magically respond to the APAP moving around all night like a rhythm guitar played in tune to the soul. I also believe that exhalation back pressure can be a bigger factor for good results than continuing changes to overall pressure, assuming there a supportive pressure programmed.
Having said all that, I believe that APAP's are good because they offer flexibility and that the P&B 420E is a very good APAP. What's good about it is that it has triggers that can be set for (or not set for) Command on Flow Limitation Run and Command on Flow Limitation combined with amplitude decrease. How either of these translates to better treatment of UARS I can't say, because I don't know, but better to have some options a person can try if they are striving for better results. Other APAPs have one program that is running for all people (cases).
I own a 420E and like it. One thing I like about it is that is has a softer back pressure than my ResMed Spirit APAP, which is the older model that doesn't have ERP.
