Remstar Auto M-Series - Problem?

Posted by todd on March 31, 2008 at 07:44:17:

I replaced my Puritan Bennet APAP a few months back with this Remstar Auto M-Series with A-Flex. It's set to APAP setting with 4 min 10 max and the A-Flex is set to 2. When I fall asleep at night I can feel the A-Flex working and when it takes awhile to fall asleep the pressure does start going up even though I'm awake and breathing on my own.

One thing is really bothering me. When I wake up every morning, the machine is on 10 and I can't feel the A-Flex doing anything at all and it's hard to breathe because of this. My previous machine sensed me awake and breathing on my own and when I woke up in the morning it'd be at the minimum setting.

I've raised this issue with my doctor and she said to ask my respiratory therapist. I raised the issue with him and he doesn't seem to get what I'm saying. He said just to push the ramp button. I can't get any support at all from Respironics.

Has anyone else noticed their machine working like this? Is this normal behavior? I'd think it should sense that I'm breathing on my own and lower the pressure. I can lay in bed for 10-20 minutes breathing on my own and it stays at 10.0 with no exhale pressure relief. This makes me wonder if the machine only has an "up" pressure adjustment when apneas are present but never goes down when apneas subside. I'm waking up tired and groggy too and so I'm concerned. It acts as if it switched over to CPAP at 10.0 during the night somehow. It has a setting for that, but it is NOT turned on. (It has a setting for 50/50 Apap/Cpap, but this feature is off.)

Shouldn't the machine go up and down in pressure during the night as conditions change?

I sent my data card to my respiratory therapist and he said it's working ok, but I didn't get to see a readout of a single night in detail so I don't know what he means by that.

Anybody who uses this machine, could you please comment on what you notice in the morning when you wake up before you turn off the machine? Is the pressure at maximum and does it stay there even when you're awake?

Thanks!
