You're on the right track seeing the sleep dentist in two weeks. I would recommend having an appliance made if the dentist feels it will work for you. Separately but involved with this, you can't ignore what you eat before bedtime and your daily exercise level. These are both items of sleep hygine - daily practices that promote better sleep. Cut back on computer time too. I would think that sucess in your case will be to fine tune everything in your life until you can achieve better sleep. CPAP is always an option in the background, even though you 'hate' it.
