It is not uncommon to have more that one sleep disorder. My bet would be insomnia induced by sleep apnea. Once the sleep apnea is treated the insomnia usually goes away and the evening sleepiness goes away.

Sleep apnea needs to be addressed first. Schedule a consult with a sleep specialist in your area. Armed with your medical history and looking at your facial features, airway, etc. he will be able to better determine the next step.

Sleep apnea has serious consequences on many different levels so for me not seeing a sleep specialist wouldn't be an option.
