Have you tried a "lower mandibular positioning appliance" like TAP-2? It's much cheaper than surgery and you reverse it every morning when you pop out the appliance.

When the tool you've got is a hammer, everything looks like a nail ... if you talk to a surgeon s/he is going to recommend surgery. Talk to your dentist about it.
