You should also consider whether your tongue is part of the obstruction. My pressures were very high, and in fact a BiPap was actually prescribed for me at one point because of the high pressures. What I eventually discovered was that the tongue was falling into the top of my esophagus, blocking the airway. And the increased pressure just clamped it down harder. IF your problem is all or partly related to your tongue, then a lower mandibular positioning appliance might alleviate the problem enough so that you can use much lower air pressures. My pressures had ratcheted up to almost 20 and now that I am using the TAP-2 my VPAP usually runs at 8-10 and is quite comfortable.
