Just so you know, oral sleep appliances can make TMJ worse.

From everything you describe it sounds like an upper airway resistance syndrome problem. With UARS you don't have full blown apnea. In fact, without monitoring esophageal pressure, with a tube down the nose, most labs will not pick it up because breathing appears to be normal.

What's happening is a narrowing of the airway and you need to make extra effort to breath. This extra effort will maintain airflow so not apneas will appear on the recording. However, the extra effort to breath will keep you from getting a normal sleep and could be causing your squeaking sound. It can also cause the bruxism (teeth grinding).

Look for a lab in your area that tests for UARS. Some labs may call it RERA's, however, there is a slight difference. If they can test for actual effort to breath and not just abdominal movement they would be able to pick it up.

UARS can have the same long term negative effects as full blown sleep apnea so you should really have it checked out.
