Yes, you can have an apnea index of 0 and have UARS. In fact that is classic UARS. With UARS the events last much longer than Obstructive events so the arousal index will be much lower than someone with OSA.
With UARS there is a slow steady increase in endoesophageal pressure. During the event you have to make more and more effort to keep breathing until the effort is to the point that you have an arousal and it starts all over again. Events can last for up up 10 minutes or more. Varies from person to person.
With some patients UARS can be more debilitating than full blown OSA.
