This may be technically illegal (at least in the U.S.). For instance, let's say a doctor "officially" charges $100 but accepts the $80 co-payment and "waives" the $20 co-payment. In reality, he is really charging only $80, so the 80% insurance coverage should only pay $64, not $80!
