There is a big difference between holding your breath during wake and
having an apnea. There are changes in blood chemistry, CO2 and O2 will
change levels during sleep, etc. When you are awake holding your breath
you are not making effort to breath, during sleep you are making a huge
effort to breath which burns off oxygen quickly.

This difference shows up with pure central sleep apnea patients, CSA vs
OSA. CSA patients can have a 2 minute apnea with very little O2
desaturation. Take a patient with typical OSA and observe a 2 minute apnea
and they can desaturate below 50%.

There is also a significant delay in most oximeters so it could take 30
seconds or longer after a blood O2 desat before you will see in reported on
the device.
