"Forehead pads should be in the middle of your forehead. To minimize leaking, I wear my mask rather low and the pads are practically on my eyebrows."

Mine rest about 1.5 cm above my eyebrows and I think it fits great.

"If it should rest in the middle of my forehead I will start bugging my supplier and medical plan to get the extension pipe."

Don't know if that's the answer, but I'd sure try it. It's worth all the effort it takes to make a CPAP as comfortable as can be.

"Also, I see that the Port Plug is a replaceable part. What purpose does it serve?"

If you're talking about the crescent shaped piece with 4 holes, it's for exhaling air.

"On the average, how often do the pads and cushions require replacement?"

I replace mine everytime I lose one! Right away I saw those little piece as a potentional problem and ordered extras off the internet.
