I worked for a dental insurance company for 8 years and have to tell you that the squeaky wheel gets the oil. Even things that were clearly not covered were occasionaly covered if the person was a big enough pain in the (take your choice)

Sleep deprivation is dangerous to you and those around you. Recent studies are showing this more and more. If your HMO has durable medical equipment coverage then you should make a big stink until they honor the contract.
